APPOINTMENTS

Your scheduled appointment time has been reserved specifically for you. We request a 24-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation.

PATIENT-CENTERED MEDICAL HOME

Patient-Centered Medical Home means that we are available during and after office hours:

  • We want you to see the same provider at every visit; however, your own provider may be fully booked. Therefore, we will assist you in seeing one of our other providers to avoid delays.
  • We routinely have evening hours at some of our locations.
  • We keep several appointments open for each provider for same-day appointments.
  • We offer appropriate medical advice and information during office hours.
  • We have a Nurse on Call (864-582-2411) after hours and weekends when the office is closed.

Patient-Centered Medical Home means we use "Evidenced Based Care” for short and long-term illnesses:

  • We care for short-term illnesses and manage long-term chronic diseases. We also offer Dental Services, in addition to offering prescription services through our Pharmacy.
  • Behavioral Health Counseling (BHC) is offered to New Patients who express an interest in seeing a ReGenesis Health Care (RHC) Behavioral Health specialist by referral after an initial NEW PATIENT visit has been completed with an RHC Primary Care Provider.
  • BHC is also available to active, established patients by requesting a referral from their PCP during a visit, by phone message to their PCP/nursing staff, or by requesting an appointment from the front desk staff.

Patient-Centered Medical Home means we organize your health care outside our practice:

  • We keep track of Lab Tests, Imaging Studies, and Referrals ordered for you.
  • We help you make specialist appointments and monitor to see if you have gone to these appointments.
  • We have processes to ensure all test results return to your provider for review.
  • We obtain records from ER visits and Hospital stays so your records remain updated. You may contact the Health Information Department at 864-582-2817 ext. 3615 if you need to obtain your medical records.

Patient-Centered Medical Home means we expect you to be an active participant in the self-management of your healthcare:

  • You will agree to keep all scheduled appointments at our office and with any specialists.
  • You will be open and honest in providing your doctor with your health-related information.
  • You will participate in developing an action plan to assist with self-management care.
  • You will notify us if your insurance, prescription coverage or financial situation changes.

Patient-Centered Medical Home means we actively seek to improve the quality of our services:

  • We respect your privacy and keep your patient health information confidential.
  • We ask for your input for improvement efforts through suggestion boxes and satisfaction surveys.
  • We are committed to providing quality, affordable care to everyone regardless of their ability to pay and no patient will be denied health services due to an individual's inability to pay for such services.
  • To ensure access to care, RHC provides services at a discounted (sliding fee) rate to patients whose total household income falls within certain parameters of the Federal Poverty Guidelines as published by the US Department of Health and Human Services. Any patient desiring to apply for health insurance can visit www.healthcare.gov to review their options.

How else can we use or share your health information?
We are allowed or required to share your information in other ways – usually in ways that contribute to the public good, such as public health and research. Our Responsibilities

  • We are required by law to maintain the privacy and security of your protected health information.
  • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.

BILLING & INSURANCE

ReGenesis Health Care is committed to providing affordable quality care to all people regardless of the ability to pay. At ReGenesis, we’re focused on the care of our patients before anything else. That’s why we offer payment options that work for everyone, including the insured, underinsured, and uninsured individuals.

Dedication to exceptional care at a rate that’s affordable is the core of our facility’s values. That’s why we accept most commercial insurance plans and we offer discount programs that makes health care affordable for all. Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them.

To ensure access to care, RHC provides services at a discounted (sliding fee) rate to patients whose total household income falls within certain parameters of the Federal Poverty Guidelines (FPG) as published by the US Department of Health and Human Services (HHS).  Our patients are able to apply for these discounted services based on family size and income that will qualify them to only pay a nominal fee for all services, including the pharmacy. In order to qualify for this program you will be asked to provide proof of income for each person living in your household. Proof of income may include a current tax return or recent pay stubs. If you’re unemployed, please bring in your last statement from the Unemployment/Employment Security Office or the Social Security Administration. Our staff can help you determine the best proof of income for your situation.  Any patient desiring to apply for health insurance can visit https://www.healthcare.gov/ to review their options.

We accept a majority of insurance plans from providers like veterans’ and military insurance, commercial insurance carriers, and Medicare/Medicaid. We file claims on behalf of all patients as a courtesy. Financial responsibility rests with the patient, so make sure that the information we have is always current.

Make sure you bring the following to your scheduled appointment:
  • Photo ID
  • Social Security Card
  • Medicare / Medicaid Card (co-pay if required)
  • Insurance Card / Supplement Insurance (co-pay if required)
  • Immunization Records (if possible)
We want you to know that we’re dedicated to providing care to those who might not be able to afford it otherwise. We offer numerous payment options including the option make payments or seek out discounts based on your finances. Please know that we’re dedicated to our patients and want for them to seek out the medical care they need at a rate that works for them; therefore, don’t hesitate to contact us just because you think you can’t afford health services.
Patient Transportation Rules

ReGenesis Health Care is committed to meeting the health care needs of our patients, and removing or reducing barriers to ensure all patients have access to available services. Transportation is offered to patients and their support persons and/or children, provided by RHC in situations where they would not have another way of attending their appointments, or picking up medications.

Transportation is available to patients of ReGenesis Health Care:

  • who are aged 18 years and over, unless they are a parent or legal guardian of a patient;
  • who are under the age of 18, when accompanied by a parent or legal guardian.

 

Transportation Rules:

1. Seatbelts, or child safety restraint seats (per applicable state laws) are required for all drivers and passengers.
2. Patients must be ready for departure at the set Transportation Appointment time. If a patient is tardy two or more times, then they may not be able to use ReGenesis Health Care Transportation Services, for up to three months.

3. No food or beverages, other than bottled water, are allowed in vehicles.

4. A ReGenesis Health Care employee has the right to refuse to transport anyone who is suspected of intoxication or under the influence of alcohol or drugs, or anyone who is distracting the driver.

5. If a ReGenesis employee providing transport feels threatened or at risk of harm before or during the transport, they have the right to deny or cease the transport.

Examples of violent and or aggressive behavior or at-risk behaviors include, but are limited to verbal, physical, or psychological abuse; intimidation; punching; scratching; biting; grabbing; pushing; threats; attacks with or without a weapon; throwing objects; sexual harassment or assault; and any other form of indecent physical contact.

These rules will be or have been reviewed with you over-the-phone during your Transportation Appointment Confirmation, as well.
