ESSENTIAL JOB FUNCTIONS

Provides hands-on dental care while achieving the highest levels of appropriateness, quality, efficiency, accessibility and responsiveness.

Performance expectations:

  • Examine individuals requesting care, diagnoses their dental/oral conditions, prescribes and carries out, or directs others in carrying out, appropriate dental/oral treatment, or refers individuals for specialty consultation or treatment in conformance with approved clinical protocols and guidelines.
  • Utilize up-to-date and Evidence-Based diagnostic and treatment techniques to provide full range of general dental services within the dentist’s defined scope of training

Support the development of ReGenesis Health Care’s department of family dentistry

Dental Hygienist

Essential Job Functions

Evaluates patients and implements treatment regimens under the supervision of the Dentist. Provides complete documentation of all findings, treatments and outcomes.

 

Performance expectations:

  • Assesses dental conditions and risk, uses patient screening procedures, including medical history review, dental charting and perio-charting; takes patient vital signs as required.
  • Delivers direct patient care to patients as assigned, using established dental hygienist procedures, such as cleaning, polishing, x-rays, application of sealants, fluoride varnishes, and related procedures.
  • Develops and implements individualized dental care plans for patients.
  • Performs patient education and risk protocols, re-care planning, and patient/family teaching under the supervision of a Dentist.
  • Initiates oral health education and prevention activities.
  • Documents dental history of chief complaint; records and reports pertinent observations and patient reactions to dental staff, as appropriate; documents lab procedures and ensure follow-up on result.
  • Assists with or institutes emergency measures for sudden adverse development during treatment of patients.
  • Assumes responsibility for the upkeep of the dental operatory and equipment.

Supports delivery of dental services to all patients of the office.

Dental Hygienist

Performance expectations:

  • Initiates patient care as appropriate for walk-in patients.
  • Assists in the preparation of patient care areas, and in the patient admission, transfer and discharge process, as required; prepares reports and assists as required with patient reception, telephone calls, routine triage and other office duties.\
  • Scans reception room for patients.
  • When needed, provides chair-side assistance to the Dentist in the performance of special tests, procedures and complex treatments.
  • Makes impressions of patients’ teeth for study casts.
  • Performs routine cleaning and maintenance of dental equipment in assigned operators and for the assigned equipment. Regularly and thoroughly performs maintenance of assigned equipment.
  • Uses downtime to clean and restock operators and prepare for future scheduled patients.

Supports the growth and development of the dental practice

Performance expectations:

  • Participates in Quality Improvement activities as requested by the Director of Dental Services.
  • Attends and participates in staff meeting to review team progress in meeting financial and productivity goals for the dental program.
  • Offers ideas for improvement of and solutions to obstacles hindering provision of patient care and desired patient outcomes as needed.

  • Minimum of an Associate’s Degree in Dental Hygiene; Bachelor’s Degree preferred.
  • Registered Dental Hygienist (RDH) from an accredited program with a current, unrestricted license to practice in the State of South Carolina.
  • CPR Certification.
  • Must have current CEUs and pursues appropriate CEUs as required to maintain licensure and stay abreast of advances in the field of dentistry, especially in areas related to the scope of dental hygiene practice.
  • At least 2 years’ experience in clinical setting as dental hygienist. Prior FQHC experience a plus.

Performance Expectations

Performs selected clinical support and administrative duties, and assists medical providers as part of the clinical care team.

Performance Expectations
  • Perform all activities related to preparation, dispensing, and sale of prescription and pharmacy-related products.
  • Assure accuracy and appropriateness of each prescription for each patient
  • Counsel patients on the proper use of their medications.

PURPOSE: Serves to provide patient care and act as leader of team of caregivers including medical, nursing, and administrative staff. Responsible for ensuring continuous quality improvement and for providing high quality care with a Patient Centered approach.

• Act as primary care provider for a panel of patients assigned by ReGenesis Health Care, providing services on premises and referring externally as appropriate.

  • Participate in the performance improvement activities of the health center that relate to the clinical program, to include the clinical care measures.
  • Participate in the call rotation with other providers at ReGenesis.
  • Accurately, and in a timely manner, complete medical records and submit bills for every patient encountered.
  • Complete peer review process every quarter.
Requirements

Qualifications:

Education- Graduate of an accredited medical school with a degree of Doctor of Medicine or Osteopathy; completion of an approved residency program in internal medicine, family practice, or pediatrics.

Qualifications:

  • Must have active pharmacist license in state of practice with all held licenses in good standing and be immunization certified.
  • Access to and familiarity with the SC PMP program

Duties and Responsibilities:

  • Work within the established organizational baseline of integrated primary health and behavioral health capability.
  • Provide direct care, to provide assessment of client needs, counseling and access to needed and appropriate services including treatment, rehabilitation and service linkage, case monitoring and client advocacy functions.
  • Maintains confidential records on client appointments, interactions and interventions and prepares related reports;
  • Reviews individual service plans, follow up and intervenes as required;

Qualifications:

Patient Transportation Specialist-Driver

Position Summary:

Under the general direction of the VP of Strategy and Development the Patient Transportation Specialist (PTS) driver accurately and efficiently transport patients. Ensuring patients of ReGenesis Health Care are provided with reliable and courteous transportation with safety as a top priority and to maintain vehicle records. This position drives health centers vehicles, interacts with patients, staff, and public in a calm, courteous, and confident manner and makes decisions as the situation demands. Maintains pleasant, friendly, professional demeanor with patients and staff always. All employees of ReGenesis Healthcare are required to follow all organization policy and procedures.

Duties & Responsibilities:

  • Operate assigned vehicle in a safe and courteous manner.
  • Maintain defensive driving training.
  • Provide a communication link between customers and staff
  • Maintain accurate, up-to-date records on trip sheets, patient transportation forms, vehicle maintenance, fuel purchases, incident reports, accident reports, and other records that are requested from leadership.
  • Provide continuous communication between patient and the health center staff. Report any change in schedule including any patient cancellations.
  • Read and interpret maps and driving directions to plan the most efficient route service for patients,
  • Ensure patient is properly identified before transporting.
  • Assist passengers, including handicapped, in and out of the vehicle; operates ramp and lift, escort to health center lobby, and open doors for them as needed
  • Performs daily vehicle checks and maintains van ensuring that it is mechanically safe.
  • Fuel the assigned vehicle adequately.
  • Works flexible shifts as work dictates.
  • Performs other duties as assigned.
Transportation Specialist - Driver

Education/Experience/Requirements

  • High School Diploma or GED
  • Certification in CPR
  • Defensive Driving
  • 4+ years of driving experience required.
  • Bi-lingual English/Spanish preferred; must be able to read, write and speak English.
