ESSENTIAL JOB FUNCTIONS

Provides hands-on dental care while achieving the highest levels of appropriateness, quality, efficiency, accessibility and responsiveness.

Performance expectations:

  • Examine individuals requesting care, diagnoses their dental/oral conditions, prescribes and carries out, or directs others in carrying out, appropriate dental/oral treatment, or refers individuals for specialty consultation or treatment in conformance with approved clinical protocols and guidelines.

Support the development of ReGenesis Health Care’s department of family dentistry

  • Assist in the provision of oral health education to the community through RHC sponsored or endorsed programs

  • Serve on the dental staff and on other RHC committees as assigned

Dental Hygienist

Essential Job Functions

Evaluates patients and implements treatment regimens under the supervision of the Dentist. Provides complete documentation of all findings, treatments and outcomes.

 

Performance expectations:

  • Assesses dental conditions and risk, uses patient screening procedures, including medical history review, dental charting and perio-charting; takes patient vital signs as required.
  • Delivers direct patient care to patients as assigned, using established dental hygienist procedures, such as cleaning, polishing, x-rays, application of sealants, fluoride varnishes, and related procedures.
  • Develops and implements individualized dental care plans for patients.
  • Performs patient education and risk protocols, re-care planning, and patient/family teaching under the supervision of a Dentist.
  • Initiates oral health education and prevention activities.
  • Documents dental history of chief complaint; records and reports pertinent observations and patient reactions to dental staff, as appropriate; documents lab procedures and ensure follow-up on result.
  • Assists with or institutes emergency measures for sudden adverse development during treatment of patients.

Dental Hygienist

Performance expectations:

  • Initiates patient care as appropriate for walk-in patients.
  • Assists in the preparation of patient care areas, and in the patient admission, transfer and discharge process, as required; prepares reports and assists as required with patient reception, telephone calls, routine triage and other office duties.\
  • Scans reception room for patients.
  • When needed, provides chair-side assistance to the Dentist in the performance of special tests, procedures and complex treatments.
  • Makes impressions of patients’ teeth for study casts.
  • Performs routine cleaning and maintenance of dental equipment in assigned operators and for the assigned equipment. Regularly and thoroughly performs maintenance of assigned equipment.
  • Uses downtime to clean and restock operators and prepare for future scheduled patients.

Supports the growth and development of the dental practice

Salary Range: Up To 115,000.00 Commisurate with experience and skill sets.

PURPOSE: Nurse Practitioner works as part of the clinical provider team performing a variety of direct patient services including assessment, diagnosis, implementation of treatment, evaluation, and monitoring substance use disorder patients management plans for the clinic patients.

The nurse practitioner may diagnose any chronic, subacute, or acute condition within the scope of family medicine and may initiate, continue, or modify therapy appropriately related to these diagnoses, with the intent of improving the overall condition of the patient.   

A Nurse Practitioner performs these functions under a current Collaborative Agreement with a licensed physician in a primary health care setting within a Federally Qualified Community Health Center, following the Health Resources and Services Administration (HRSA) guidelines.

Purpose, Duties and Responsibilities

Duties and Responsibilities:

Diagnoses and treats acute health problems, treats and manages chronic diseases;

Orders, performs and interprets diagnostic studies; Prescribes medications and other treatments as necessary for adult and pediatric patients.

Obtains medical histories and provides health maintenance including physical examinations;

Interprets and makes appropriate decisions based on pertinent laboratory and clinical findings

Promotes positive health behaviors through education and counseling.

Collaborates with physicians and other health care professionals as necessary.

Participates in organizational meetings and collaboration with internal providers.

Complete timely documentation in the EMR for each patient encounter and review patient medical records for accuracy, and completeness as outlined in policy for ensuring timely medical record documentation.

Prescribes treatment plan for management/care for patients and families according to protocols and refers appropriately.

Develops a health maintenance plan for individual patients which includes health education, health promotion/disease prevention and general anticipatory guidance.

Counsels patients and families appropriately regarding management of their care.

Demonstrates proficiency in all the above.

Confer with supervising physician at least monthly, or when needed for consultation, information, etc.

Participates in quality improvement activities – review of medical records for adherence to pre-established standards of care, participation in quality improvement meetings and processes that involve Patient-Centered Medical Home and our Federally Qualified Health Center status, and participation in provider meetings.

Enhances acumen and practical knowledge of quality initiatives through reading and participation in seminars, workshops.   Maintain appropriate licensure and participate continuing education to perform the duties of a Nurse Practitioner in South Carolina.

Required to observe all OSHA and Health Center requirements.

Educational requirements:

Must have a graduate degree in Counseling from an accredited college or university.   • Perform all activities related to preparation, dispensing, and sale of prescription and pharmacy-related products.
  • Assure accuracy and appropriateness of each prescription for each patient
  • Counsel patients on the proper use of their medications.

Director of Behavioral Health

PURPOSE: The Director of Behavioral Health serves to provide leadership in all areas relating to the behavioral health department of ReGenesis Health Care, Inc. (RHC). The Director of Behavioral Health is responsible for recommending and implementing behavioral health policies and procedures; supervising the behavioral health staff; insuring the continuous quality improvement program is administered in all behavioral health activities; and participating as an integral part of the clinical care and leadership team; Performs the functions of a Behavioral Counselor within interdisciplinary, integrated team, within Federally Qualified Community Health Center, located in Spartanburg and Cherokee counties; following the Health Resources and Services Administration (HRSA) guidelines.

 

Duties and Responsibilities:

Directs the behavioral health programs of the organization according to the guidelines established by the CMO.

Supervises the performance improvement activities of the health center that relate to the behavioral health program, to include the clinical care measures.

Establishes an organizational baseline of integrated primary health and behavioral health capability.

 

Ensure the development, implementation and delivery of:

 

  • a formal communication process to inform about ongoing integration efforts at the system, team, and individual level;

 

  • a system that supports face-to-face and team meetings to discuss patient care;

 

  • An integrated model supporting where all patients have a treatment plan;

 

  • Identification and use of universal, population-based screening;

 

  • Educating internally to garner organizational support of integration

 

  • Maximization of billing to support the program;

 

  • Recommends hiring strategies to maximize patient care and revenues while minimizing costs for the organization;

 

  • Community relations to include robust referral networks and partnerships to support overall health of patients;

 

  • A plan to have regular access to a psychiatric consultant;

 

  • A policy manual, or integrate behavioral health language into existing policies, to structure the program and inform future RHC stakeholders;

Behavioral Counselor

As a leader, maintains an active patient panel, providing face-to-face behavioral health care, maintaining or exceeding the same expectations of productivity and quality expected of supervisees during time not allotted for administrative duties.

Maintain direct care, to provide assessment of client needs, counseling and access to needed and appropriate services including treatment, rehabilitation and service linkage, case monitoring and client advocacy functions.

Maintains confidential records on client appointments, interactions and interventions and prepares related reports;

Reviews individual service plans, follow up and intervenes as required;

Participates in routine and special clinical supervision, staffing, consultations, training and related meetings and seminars; Attends interdisciplinary team meetings to discuss decisions for client plan of care;

Consults with representatives of other agencies or programs to coordinate client treatment services and case management services;

Performs discharge and aftercare planning; Performs follow-up with clients to determine their progress and adjustment during and after services.

Enhances acumen and practical knowledge of quality initiatives through reading and participation in seminars, workshops. Maintain appropriate licensure and participate continuing education to perform the duties of Behavioral Counselor in South Carolina.

PURPOSE: Promote ReGenesis Health Care services to target populations within Union, Spartanburg, and Cherokee counties by providing presentations, marketing materials and referrals to local resources – primarily in Union County.

 

Duties and Responsibilities:

  • Coordinate and attend community events and design public education activities to raise awareness about ReGenesis Health Care and the services provided, as well as the full range of available health insurance coverage options.
  • Serve as liaison to public community organizations, nonprofits, public partners and stakeholders to further strengthen RHC’s outreach strategy.
  • Provide application and renewal assistance to social service programs, i.e. Medicaid, SNAP, ACA Marketplace and other health-related benefits, while maintaining knowledge and expertise on ongoing changes in eligibility, enrollment and program specifications to inform and assist RHC patient population.
  • Understand and maintain a professional relationship while also providing culturally competent customer service.
  • Collects statistical information from presentations and enrollments.
  • Provides weekly report regarding outreach and application assistance provided to the community and RHC patients to the Director of Outreach Development.
  • Demonstrate and maintain the standards and requirements of the Health Insurance Portability and Accountability Act (HIPAA); while insuring the protection and security of personal, confidential and identifiable information in a professional and responsible manner and carry out all measures to prevent from unauthorized disclosures.
